How Humana usually covers physical therapy

Many Humana plans include coverage for physical therapy when it treats a specific problem, such as back or neck pain, an injury, or recovery after surgery. What you actually pay depends on your plan: the yearly visit limit, your copay or coinsurance, and how much of your deductible is left.
